At The Health Plan, we have a team of Health coaches, Social Workers, and Member Advocates on staff. The provide support and education to our members with topics including:
- Healthy weight management
- Encouraging physical activity
- Managing stress
- Identifying depressive symptoms
- Avoiding atrisk behaviors
- Smoking or Tobacco Cessation
They can also direct you to tools, resources, and services in your community to help meet your needs. This includes:
- Food resources
- Housing support
- Other resources to keep you healthy and maximize your benefits
Our CoreWellness program is an individualized program that helps you to build and maintain healthy habits. If interested, a certified health coach will contact you and help you determine realistic, and attainable, health goals. We’ll give you small, but manageable, changes that you can make to build and maintain new habits. Follow up calls are made throughout your program to give more education and support as you need it. After you’ve met your goals, you’ll get a 1 month and 3 month post-program call to see how your maintaining your goals or help you with slip ups. One of the most important actions you can take to keep yourself healthy is to quit using tobacco. All tobacco users are encouraged to quit. If you are interested in tobacco cessation agents, you will need to see your physician for a prescription. The Health Plan offers members a tobacco cessation program to help you stop smoking, rubbing, or chewing. Coverage for the tobacco cessation program is free for most members. Coverage for self-funded members may vary.
