Your prescription benefit may provide coverage for some products as preventive medications. Coverage of these products may be limited to certain formulary drugs as determined by The Health Plan.

The preventive items that may be included on your pharmacy coverage with a prescription includes:

• Fluoride: single entity oral dosage forms - 0.25mg & 0.5mg drops/tablets
• Aspirin: only includes OTCs; 81mg & 325mg tablets
• Smoking Cessation: nicotine replacement, Zyban & Chantix for age 18 and over. Limited to one 180-day treatment per year
• Folic Acid: only includes OTC; 0.4mg & 0.8mg tablets for women up to age 63
• Iron: only includes OTC drops up to age 1
• Contraception: includes generic oral contraceptive, injectable hormone (depo-provera), and vaginal ring (Nuvaring)
