Eligibility

 

To be a member of one of our Plans, you must live in our service area, be entitled to Medicare Part A, and enrolled in Medicare Part B, and not have End Stage Renal Disease (ESRD), with limited exceptions, such as if you are already a member in one of our fully-insured plans. If you currently pay a premium for Medicare Part A and/or Medicare Part B, you must continue paying your premium in order to keep your Medicare Part A and/or Medicare Part B and remain a member of this plan.

The Health Plan SecureCare Plan (HMO) is offered to the residents of the following:

OHIO COUNTIES in the HomeTown Region: Ashland, Carroll, Columbiana, Coshocton, Holmes, Mahoning, Medina, Portage, Stark, Summit, Trumbull, Tuscarawas, and Wayne counties.

OHIO COUNTIES in the Ohio Valley Region: Belmont, Guernsey, Harrison, Jefferson, Noble, Monroe, Muskingum, and Washington counties.

WEST VIRGINIA COUNTIES in the Ohio Valley Region: Brooke, Hancock, Marshall, Ohio, Tyler, and Wetzel counties.

WEST VIRGINIA COUNTIES in the Mountaineer Region: Barbour, Calhoun, Doddridge, Gilmer, Harrison (WV), Lewis, Marion, Monongalia, Pleasants, Preston, Randolph, Ritchie, Taylor, Tucker, Upshur, Webster, Wirt and Wood counties.

THP SecureChoice Plan (PPO) is offered to the residents of the following:

Enrollment

• You will need to keep Medicare Parts A and B and you must continue to pay your Medicare Part B premium, even if the plan premium is $0.
• The Health Plan’s contract with CMS (Medicare/Medicaid Services), is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed.
• THP Insurance Company's contract with CMS (Medicare/Medicaid Services), is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed.

Enroll By Phone:

  Call us at 1.877.847.7907 (Ohio Valley or Mountaineer Region) or 1.877.236.2290 (HomeTown Region).

Enroll Online:

You may enroll electronically. After completing the online application, you will receive a confirmation number. This number verifies that you have chosen to enroll and Medicare will notify The Health Plan/THP of your enrollment. You will be redirected to the Medicare website to complete an enrollment application.

Medicare beneficiaries may enroll in The Health Plan SecureCareHMO or THP SecureChoicePPO through the CMS Medicare Online Enrollment Center.

For SecureCareHMO:

I understand that beginning on the date The Health Plan SecureCareHMO coverage begins, I must use plan providers except in emergent or urgent care situations or for out-of-area renal dialysis. If I obtain routine care from out-of-network providers neither Medicare nor The Health Plan will be responsible for the costs. Services authorized by The Health Plan/THP and other services contained in my SecureCareHMO Evidence of Coverage document (also known as a member contract or subscriber agreement) will be covered. Without authorization, NEITHER MEDICARE NOR THE HEALTH PLAN WILL PAY FOR THE SERVICES.

For SecureChoicePPO:

I understand that when THP Insurance Company SecureChoicePPO coverage begins, using services in-network can cost less than using services out-of-network, with the exception of emergency or urgently needed services or out-of-area dialysis services. THP Insurance Company provides reimbursement for all covered benefits regardless of whether they are received in-network as long as they are medically necessary. Services authorized by The Health Plan/THP Insurance Company and other services contained in my SecureChoicePPO Evidence of Coverage document (also known as a member contract or subscriber agreement) will be covered. Without authorization, NEITHER MEDICARE NOR THP INSURANCE COMPANY WILL PAY FOR THE SERVICES.

Changing your method of premium payment: If you decide to switch to premium withhold or move from premium withhold to direct bill, it could take up to three months for it to take effect and you will be ultimately held responsible for those premiums.
