The Health Plan provides coverage for your diabetes testing needs with the Lifescan® products described below. If you currently do not use one of these meters you need to contact your physician to order the Lifescan® meter below that fits your needs. Members are provided one meter per lifetime at no charge.

In addition to OneTouch® glucose testing supplies, you may have coverage for lancets, alcohol swabs and other supplies needed for diabetes management. Check your Schedule of Benefits for coverage. All of your covered supplies can be obtained through any participating pharmacy including Medco Mail. Additionally, you may order the meter of your choice by calling Lifescan at 1.877.493.8378 and mention Order Code: 646THP002.
In certain cases it may be necessary for you to continue using your present blood glucose meter system, if so, have your physician contact Pharmacy Services for a coverage review. The Health Plan has registered nurses who are Certified Diabetes Educators (CDEs) in the Disease Management Department and who can coach you to better health with better control of your diabetes.
