Attention: Members with Diabetes

The Health Plan provides coverage for your diabetes testing needs with the Lifescan® products described below. If you currently do not use one of these meters you need to contact your physician to order the Lifescan® meter below that fits your needs. Members are provided one meter per lifetime at no charge.

	The OneTouch® UltraMini® Meter offers a simple way to check your blood sugar on-the-go. Available in five great colors - Limelight®, Pink Glow®, Silver Moon®, Purple Twilight®, and Blue Comet®.
	The OneTouch® Ultra®2 Meter is a fast, gentle and simple way to help you see the effects of food on your blood glucose results.
	The OneTouch® UltraSmart® Meter is for people making insulin adjustments and is the only blood glucose meter proven to help you lower A1c levels. ¹
	The OneTouch® UltraLink Meter wirelessly communicates to the MiniMed Paradigm® insulin pump, making bolus dosing easier and more accurate. ²

In addition to OneTouch® glucose testing supplies, you may have coverage for lancets, alcohol swabs and other supplies needed for diabetes management. Check your Schedule of Benefits for coverage. All of your covered supplies can be obtained through any participating pharmacy including Medco Mail. Additionally, you may order the meter of your choice by calling Lifescan at 1.877.493.8378 and mention Order Code: 646THP002.

The Health Plan has registered nurses who are Certified Diabetes Educators (CDEs) in the Disease Management Department and who can coach you to better health with better control of your diabetes.
