At The Health Plan, our benefits and programs encourage people to stay healthy by seeking preventive care and medical treatment at the onset of illness. We focus on wellness programs to promote healthy lifestyles and minimize health problems, enhancing the quality of life of our enrollees.

We invite you to take advantage of the wealth of information and services offered here to enhance your well-being. Listed below are a number of links to various wellness and disease management sites.

If you choose to use any of the links we provide, you will be leaving our website and going to a new website. Health Risk Assessments

The Health Plan is pleased to present online Health Risk Assessment tests from HealthStatus. You can get customized health reports and use our in-depth health calculators. Sign up today to get started!

HRA Disclosure

The information gathered as part of the Health Risk Assessments is confidential. In order to better assist our members with their healthcare needs, the information provided may be used by The Health Plan for the following purposes:

  • to provide outreach services to members in need of services;
  • to provide outreach services to members in need of education;
  • to provide group reporting in order to determine the need for additional disease management programs by The Health Plan or individual employers;
  • to disclose information to a member's PCP (Primary Care Physician) ;
  • other capacities as deemed appropriate for determining the healthcare needs of our members

Information is NOT used for the following purposes:

  • to determine eligibility for enrollment into The Health Plan;
  • to provide individual information to employer groups

Please be aware that the following assessment tests may take anywhere from 20 to 40 minutes to complete. Also, note that a particular service or treatment listed in the assessment is not a guarantee that it is covered under your benefit plan.
