Behavioral Health Additional Resources

Your PCP is the gatekeeper for your care, whether it is for a medical or behavioral health condition. It is best if your doctor knows all aspects of your health and care. To be sure that he is informed so that he can give you the best care, make sure that all of your doctors are keeping in touch with your PCP. At your next visit to any specialists, ask if your PCP will receive a report of the visit and any medications that you are prescribed. The specialist may ask you to sign a “Release of Information” stating that it is ok with you to send this information to your doctor.

Our staff is responsible for the entire Behavioral Health process of care. We handle your and your provider's questions, receive and evaluate requests for treatment, manage your follow-up care and process your claims.

If you need to seek treatment and have never seen a behavioral health provider, refer to your provider list to locate and make an appointment with a participating provider. If you need assistance or have any questions regarding behavioral health, please call our office at 1.877.221.9295 and we will help you. Remember, if you are covered under a self-funded program, some precertification requirements may apply. Refer to your benefit book or call The Health Plan for clarification.

Depression Disease Management

The Behavioral Health Department now offers a program to help members manage depression. If you or a family member have been diagnosed with Major Depression and would like information about this program, call the Behavioral Health Unit at 1.877.221.9295 and a nurse will call you.

   

Behavioral Health Unit

The Health Plan Behavioral Health Unit offers Case Management Services to our members. We want to help you and your family manage not only the symptoms of your illness, but any other associated problems that you are having.

The case manager will be available to help you with questions about your illness , your medications and your treatment, but, also, is here to help you with adjusting to life in the community. If you are having problems affecting your housing, transportation, applying for programs, getting supplies, attending appointments for psychiatrists or therapists or affecting employment, you may qualify for case management.

Our goal is to help you by trying to connect you with the services that you need. Our case manager is available by phone at 1.877.221.9295 (24 hours per day).

This form can be filled out online and submitted for more information on topics such as Alcohol, Depression, Stress Management, and Substance Abuse. A Case Manager will respond to your request on the next business day.

   

Advance Directive for Behavioral Health

An advance directive is a legal document that tells what healthcare services you want to receive if an illness makes you unable to make decisions for yourself. View our Behavioral Health Advance Directive brochure for  more information.

Each state has different rules about how advance directives must be written and approved. To make sure you have all the information you need, check with your state’s protection and advocacy program, a lawyer, paralegal or advocate. Please note that only attorneys can offer legal advice.
