The Health Plan’s Provider Search allows you to search online for a primary care physician or specialist in the city/area you reside and extend your search within a 25 mile radius of that area/city. Directories are also available to view and print online. If you do not have access to the Internet to take advantage of these capabilities, you can contact The Health Plan Customer Service Department in St. Clairsville at 740.695.7902 or in Massillon at 330.837.6880 to have a copy of the directory mailed to you.

In an ongoing effort to educate our members and encourage you to have preventive healthcare and screenings, we have also updated the wellness portion of our website. The Health Risk Assessment has been enhanced and continues to be used by our members. This tool is interactive and is designed to help you and your physicians determine your health risk for certain preventable diseases. It also helps you identify key habits you can change to live healthier and suggests appropriate screenings you should have. You will find links to related websites as well as a section where you can calculate BMI (body mass index), learn your ideal weight, find your target heart rate and much more.
