On January 1, 2008, an internal Behavioral Health Unit was created at The Health Plan. Although outside vendors had effectively managed behavioral health services at the plan in the past, it was felt that a greater coordination of care would be possible with the integration of behavioral services. As with other services, our goal is to ensure the highest quality of care for our members. To that end, we work directly with our providers and members to coordinate care needs. Our staff are available daily when needed to assist providers and members seeking to access available resources within the provider network and community network.
The BHU (Behavioral Health Unit) will work directly with other departments at The Health Plan to address behavioral health related concerns. This will enable us to better incorporate behavioral health components with Disease Management Programs, Case Management needs, primary care and specialty care.
Information regarding the referral process, preauthorizations, medical management oversight, forms necessary for continuation of treatment and reimbursement can be found in the Provider Manual, Chapter 8.
You can reach the Behavioral Health Unit at 1.877.221.9295 (24 hours a day) with any questions. Fax requests for treatment and reporting of crisis encounters to 740.699.6255 or 1.866.616.6255. This is a dedicated fax line that is available to the BHU staff only.
Users will need to have the most recent version of Adobe Acrobat or Adobe Reader (7.0 or greater). Users will be able to fill out the form ONLINE and click "Print Form" when finished. The information will be on the form and users can then fax or mail the completed form to The Health Plan Behavioral Health Unit. If users do not or cannot have the the lastest Adobe Acrobat or Adobe Reader, they can scroll to the bottom of the forms and click on "Print Form". The form will be printed for the user to then manually fill out and return to the Behavioral Health Unit.
Crisis Encounter Report Form (64k)
This form will aid the transportation of vital behavioral health information
from your facility to our Behavioral Health Unit. No preauthorization is
needed by the patient. Complete all appropriate information on the form and
