Wellness Services

We invite you to take advantage of the wealth of information and services offered here to enhance your well-being. Within the health and wellness pages, you will find a number of links to various wellness and disease management sites.

Behavioral Health

Our staff is responsible for the entire behavioral health process of care. We field questions from you and your provider, receive and evaluate requests for treatment, manage your follow-up care, and process your claims. If you need to seek treatment and have never seen a behavioral health provider, refer to your provider list to locate and make an appointment with a participating provider.

Case Management

For members experiencing a critical illness or injury, The Health Plan provides specially trained registered nurses in case management to assist in coordinating care and services. If you believe you could benefit from this program, you can contact the case managers by calling our Medical Department at 740.695.7644 or 740.695.7643 or toll-free at 1.800.624.6961, ext. 7644 or 7643.

You can also submit a Member Case Management Referral Form online.

Nurse Information Line

If you need someone to talk to about a health concern you may have, our nurse information line will help you to get in touch with one of our many nurses, 24 hours a day, 7 days a week.

If you need any written information provided by The Health Plan in an alternate language or format due to special needs such as vision, reading impairments, or language translation difficulties, please contact our Customer Service Department at 740.695.7902 or toll-free at 1.888.613.8385, Monday through Friday, 8:30 am to 5:00 pm.

The volume of calls we receive is typically higher on Mondays. You may receive quicker service if you call us on another day of the week. If you leave a message in our voicemail system, during regular business hours, please be aware that every attempt will be made to return your call the same day. After hours, please leave a message in our 24-hour voicemail system. Your call will be returned the next business day.
