Disease Management Program
The Health Plan’s Disease Management and Health Promotion Programs are developed to proactively identify populations with, or at risk for, chronic medical conditions. We currently manage members with diabetes, chronic heart failure (CHF), and chronic obstructive pulmonary disease (COPD). (read more . . . )
Prenatal & Pregnancy
Our Prenatal & Pregnancy Program is designed to improve pregnancy outcomes, reduce neonatal hospitalizations, and reduce all costs associated with preterm birth and other complications of pregnancy. This program provides prenatal education, promotes safe health behaviors, and enhances the management of maternity care for women identified at high-risk for premature labor and delivery.
